COMMUNITY HABILITATION
SUPPORT & SERVICES
Modest’s Community Habilitation program helps individuals learn the skills they need to live safely and more independently, meet people, make, and keep friends, take part in community events and be a part of their community. Because everyone is different and has different needs, we use a person-centered approach to increase independence and help each individual live a life full of meaningful interaction and possibilities.
Our goal is to help the individual and their family think strategically about the areas we may be able to enhance their independence. Some of these most common areas include:
- Assistance with daily living skills – health and personal safety, and personal care
- Opportunities to work on personal skills in the community
- Travel training – learning how to navigate in the community
- Health & wellness
- Self-advocacy & making informed choices
- Social skills development
- Money management
- Food preparation
- Safe use of public transportation
